All undergraduate students and health sciences graduate students (MPAM, DPT, and MSAT) are required to submit health forms and immunization records. All forms and a list of required immunizations are found on the Student Health Portal. Students can access the health portal using the same username and password as their University network account.
Students may submit requests for medical or religious exemptions for required vaccinations. Exemption forms must be uploaded to the student’s medical record through the Student Health Portal.
Full-time undergraduate students are subject to a fine of $200 for incomplete immunization records.
