Required Health Information

All undergraduate students and, health and medical sciences graduate students (MPAM, DPT, and MSAT) must submit health forms and immunization records.

Students must provide an official immunization record signed or stamped by a healthcare professional.  Immunization dates must include month, day, and year. 

Please go into the Student Health Portal, under the My Forms tab, complete all the forms, and upload your official immunization document to the Immunization Form. Students can access the health portal using the same username and password as their University network account.

You will be notified about any incomplete requirements via a secure message on your student health portal. 

Full-time undergraduate students are subject to a fine of $200 for incomplete immunization records.

Health Form Due Dates

  • Summer graduate programs: May 15
  • Fall entrance: July 15
  • Spring entrance: Jan. 15

Undergraduate Immunization Requirements

Required Immunizations
  • Tetanus, Diptheria, and Pertussis Vaccine: Tdap or Td within the last 10 years
  • Hepatitis B Vaccine: Three doses or positive titers
  • Measles, Mumps, and Rubella: Two doses or positive titers
  • Meningococcal Vaccine (types ACYW): One dose after 16th birthday
  • Polio (IPV or OPV): four doses with one being after the fourth birthday
Strongly Recommended but not Required Immunizations
  • COVID-19: per current CDC recommendations
  • HPV: Quadrivalent or bivalent
  • Hepatitis A: 2 doses
  • Meningococcal B Vaccine
  • Varicella Vaccine: Two doses one month apart for adults with no history of disease) or positive titers.

International Immunization Requirements

  • Please print this Immunization Form (PDF) and take it to your doctor to be completed and signed.
  • We recommend getting the required immunizations in your home country as they can be expensive in the United States.
  • Go to your Student Health Portal to your “My Forms” page and complete your forms.
Required Immunizations
  • Tetanus, Diptheria, and Pertussis Vaccine: Tdap or Td within the last 10 years.
  • Hepatitis B Vaccine: Three doses or positive titers (titers only if vaccine records are not available).
  • Measles, Mumps, and Rubella: Two doses or positive titers (titers only if vaccine records are not available).
  • Meningococcal Vaccine (types ACYW): One dose after 16th birthday
  • Polio (IPV or OPV): four doses with one being after the fourth birthday
  • If you live in a country with a high incidence of tuberculosis, which includes all countries except the United States, Canada, Australia, New Zealand, Northern Europe, and Western Europe, you must provide proof (within 6 months before arrival on campus) that you are free of tuberculosis. You can download this Tuberculosis Testing Form (PDF), have it completed by your doctor, and then upload it to your student health portal after it’s done.

Graduate Health and Medical Sciences Immunization Requirements

Required Immunizations
  • Tetanus, Diptheria, and Pertussis Vaccine: Tdap or Td within the last 10 years
  • Hepatitis B Vaccine: Three doses or positive titers
  • MMR Vaccine: Twp doses, one month apart, the first dose after the first birthday or positive titers
  • Varicella Vaccine: Two doses one month apart for adults with no history of disease or positive titers.
  • A 2-step PPD (Tuberculosis Testing): within six months of matriculation is required, but can be placed on campus with your program during orientation or at Health Services. 
Strongly Recommended but not Required Immunizations
    • COVID-19: per current CDC recommendations
    • HPV: Quadrivalent or bivalent
    • Hepatitis A: Two doses
    • Meningococcal B Vaccine 
    • Meningococcal Vaccine (types ACYW)
    • Poliomyelitis (OPV or IPV): date of series completion

A physical examination is not required.

Students may request medical (PDF) or religious (PDF) exemptions for required vaccinations. Exemption forms must be uploaded to the student’s medical record through the Student Health Portal.
