
Lynchburg College Student Athletes and Parents,
The following information and links are provided to assist with the paperwork necessary for student athletes to participate in intercollegiate athletics at Lynchburg College.
NEW AND RETURNING STUDENT ATHLETES - Please use the appropriate category and have all your paperwork COMPLETED in FULL prior to your preseason administrative meeting with the Athletic Training Staff.
*Important Note – The athletic department requires all NEW student athletes to have an initial pre-participation physical examination prior to participating in intercollegiate athletics. The Athletic Training Staff will verify all physicals though the Health Center at Lynchburg College. This is the same form that is used for all students at Lynchburg College as an admittance requirement. Please make sure you have completed your physical examination and immunization records with the health center and have signed the last box on page 4 labeled “For Athletes Only”. RETURNING student-athletes in good standing are not required to obtain a pre-participation physical examination after their first year of participation.

A returning athlete is to be considered a Lynchburg College Student Athlete that has participated annually without a lapse. If an athlete misses a year, they will be required to obtain a current preparticipation physical examination prior to participation. Please use the Health Center Physical Form.

NEW ADHD MEDICAL EXEMPTION REQUIREMENTS
For the fall of 2009, the NCAA has changed the medical exemption process for student athletes who are taking prescription medications for Attention Deficit Hyperactivity Disorder or ADHD. All student athletes who are taking prescription medications to treat ADHD must now have their physician complete the ADHD Medical Exemption Form found at the link below before being eligible to compete prior to their athletic season. Please be advised that the ADHD Medical Exemption Form will be labor intensive for your physician so please take that into consideration when asking your physician to complete the form.
