ATHLETIC DEPARTMENT STUDENT-ATHLETE REQUIRED FORMS
Lynchburg College Student-Athletes and Parents:
The Lynchburg College Athletic Department and the NCAA require each student-athlete to complete particular forms in order to participate on an athletic team. All forms are mandatory and are kept on file in the Director of Athletics office.
It is the responsibility of the student-athlete to complete the required forms completely and correctly, and submit to their respective head coach at the first team meeting or as instructed by the head coach. You will not be allowed to participate until all paperwork is completed and is on file with the Director of Athletics. Forms that are not completed correctly will be returned to you. Please note: Parent/Guardian signature is required if Student-Athlete is a minor.

General Amateurism and Eligibilty form for International and Select Student-Athletes:
International student-athletes are required to contact the Director of Athletics for the appropriate forms.
NCAA - Permission to Contact: Self-Release – Division III (link)
The purpose of this form is to grant Division III student-athletes permission to contact other Division III institution’s athletics department staff about a possible transfer. This form is ONLY to be completed if you wish to transfer to another institution. This form is not returned to the head coach or Lynchburg College Athletic Office.
