NEW ATHLETE FORMS –

Biographical Data Questionnaire
Parent/Student Insurance Information Form
Copy of Insurance Card
Medical History Questionnaire
Medical Authorization Form (Parent/Guardian Signature if under age 18)
Copy of Pre-participation Physical Examination (Third page of required physical from health center)
**Any important medical release from a Medical Doctor regarding illness, recent injury or surgeries.

RETURNING ATHLETE FORMS - 1) Returning Athlete Health Status Form, 2) Updated Biographical Data Form, 3) Updated Parent-Student Athlete Insurance Information Form. 

 **INCOMING FRESHMEN/TRANSFER STUDENTS- In addition to above paperwork, you must complete a Medical Waiver Form and present at tryout registration.  This form is located on Lynchburg College cheer web page.  This form is required to be completed and turned in prior to your participation.

 Athletic Department:  The 5 forms required by the athletic department can be downloaded from the Lynchburg College web page.  Once on the web page, click on “Athletics”.  The Athletics web page will come up.  In left hand column, you will see “Student –Athlete Forms”.  Click on that button and it will take you to all forms.  Just follow the instructions from there.  The forms you need to have completed prior to arriving at tryout registration are: 1) Student-Athlete Information Form, 2) Athletic Alcohol and Drug Consumption Statement, 3) Statement Prohibiting Hazing and Sexual Misconduct Policy, 4) Fitness Center Policy/Release Form, 5) Sports Information Form.  In addition to completing these forms, you should review the Lynchburg College Student-Athlete Handbook and be familiar with its location online.
