Meal Plan Exception Form

Students living in a residence hall must be enrolled in at least the 160-meal plan (default plan) while Southside residents are able to select the $500 Hornet Plan.

Students requesting a meal plan exception for non-disability (medical, dietary, physical) reasons, must complete and submit the Meal Plan Exception Application (PDF) to the Office of Residence Life.

Students requesting meal plan adjustments based on disability (medical, dietary, physical) must meet with Julia Timmons, Disability Services Coordinator (DSC), to disclose disability. These students DO NOT COMPLETE this form.

In keeping with Section 504 of the Rehabilitation Act of 1973 and the Americans with Disabilities Act of 1990 and its amendments the DSC works with students with disabilities to determine qualifications and reasonable accommodations. Accommodations appropriate to the student’s individual needs are approved on a case-by-case basis.
