Directions:
  1. Print out and sign the hazing statement.  Click here for the hazing form.
  2. Complete the fields below and press the "submit" button only once.
  3. Once the Confirmation page is displayed, print it out and acquire the appropriate signatures.
  4. Attach a new (updated) copy of your organization's constitution to your confirmation page.
  5. Return your confirmation, constitution, and anti-hazing statement (with signatures) to the Office of Student Activities.
