Documentation Criteria

Accommodations are established to provide opportunities for equal academic access and equal opportunity for students with disabilities to demonstrate their knowledge. Services vary according to the individual needs of each student and are determined on a case by case basis. In order to fully evaluate a student’s request for accommodations or auxiliary aids, the Disability Services Coordinator needs appropriate reasonable documentation of disability. 

It is the responsibility of the student to provide appropriate documentation to the DSC. The documentation must (1) verify the existence of a disability as defined under section 504 and under the ADA; and (2) establish a clear connection between the effects of the disability and the accommodations being requested. Documentation criteria (PDF) are provided as a guide to assist students in gathering this information.
