I have read and understand the technical standards listed and I believe to the best of my knowledge that I can meet each of these standards with certain accommodations. I will contact the Learning Resources Coordinator to determine what accommodations may be available. I understand that if I am unable to meet these standards with or without accommodations, I will not be admitted into the program. I also understand that regardless of the accommodations made for me I must still satisfy all Selection and Retention criteria of the Athletic Training Major.
